Informed Consent

Thank you for considering MyndStories! Before beginning therapy, it is essential to understand the nature of the services provided, your rights as a client, and the responsibilities of both you and your therapist.

These Informed Consent Terms (the “Terms”) are a binding agreement between you and MyndStories, an online platform facilitating mental health services. By registering for, booking, or accessing therapy sessions on the MyndStories platform (the “Site”), you accept these Terms and warrant that:

  1. You are at least 18 years old, or if you are under 18, you have the consent of a parent or legal guardian.
  2. You have read, understood, and agreed to the terms outlined herein.

Please read this document carefully. By proceeding with a booking, you agree to the terms outlined below.

1. Nature and Scope of Therapy

Therapy is a collaborative process aimed at facilitating self-awareness, personal growth, and emotional wellbeing. Your therapist will co-create a supportive environment to help you explore and address your challenges. However:

  • Therapy is not a quick fix or a guaranteed cure. Progress depends on your active participation.
  • Your therapist does not "heal" or "cure" you and the steps toward improvement are yours to take.
  • Therapy may involve talk therapy or other evidence-based methods but does not include the prescription of medications.
  • Therapy is not a substitute for crisis management. If you are in an immediate crisis, please contact emergency services or visit the nearest emergency or mental health facility.

2. Confidentiality

Your sessions are confidential, and your privacy is a priority.

Therapy sessions will not be audio- or video-recorded by MyndStories or your therapist. You must not record, reproduce, share or circulate therapy sessions or their contents without the other party's consent, subject to applicable law.

However, there are certain legal and ethical exceptions where confidentiality may be broken:

  • If there is a threat of harm to yourself or others.
  • In cases of suspected abuse or neglect of a minor or vulnerable adult.
  • When required by a court of law.

Your therapist will discuss these exceptions with you if they arise.

3. Professional Relationship and Boundaries

It is natural to develop an emotional connection with your therapist during therapy. However, please note:

  • The relationship is professional, not personal.
  • Boundaries are essential to maintain the therapeutic process and ensure your wellbeing.

4. Therapist’s Rights

Your therapist reserves the right to:

  • Terminate sessions if your needs fall outside their professional expertise. A referral to another therapist will be provided when possible.
  • End therapy if your behavior is rude, constitutes harassment, or violates professional boundaries.
  • Decline liability for actions you choose to take in your life, including harm to yourself or others. Therapy cannot guarantee outcomes and that clients remain responsible for their own decisions.

5. Client’s Rights

As a client, you have the right to:

  • End therapy at any time. You will need to inform your therapist of your decision.
  • Ask questions about the therapy process, including the techniques and approaches used.
  • Participate actively in creating and modifying your therapy goals.

6. Scheduling, Cancellations, and Rescheduling

  • Sessions are scheduled in advance for a set duration. Late arrivals may not result in extended sessions.
  • Cancellations or rescheduling must be made at least 48 hours in advance.
  • Cancellations within 24 hours of the session will not be eligible for a refund.

To cancel or reschedule, please contact us directly.

7. Legal Acknowledgement

By proceeding with therapy, you acknowledge that this form is legally binding and serves as proof of your agreement to these terms.
